They don't trust employees to make decisions. Employees are not empowered at all and risk taking is avoided at any and all costs. Executives micromanage every aspect of the company and are functioning about 2 grade levels below where they should be. Too often "performance" is measured by how many hours you work, not how smart you work. People are rewarded for taking heroic measures to solve problems, rather than preventing them.
