Management frequently changed direction without communicating these changes, then held staff responsible for outcomes that were never briefed. Policies were applied inconsistently and not in line with the organisation’s own standards, making it difficult to trust the process. Raising concerns felt unsafe, as support was not impartial. The result was a toxic, blame-led culture where colleagues avoided collaboration out of fear rather than lack of willingness.
