Submitted application in response to ad, but fell to the bottom of the HR slush pile. Used LinkedIn to identify and contact actual hiring manager. The interview was over breakfast in the manager's home town (he worked remotely). Very informal.

the hiring manager held all the cards (as it should be) - turn the hiring manager into your friend and advocate because they are the one that will go to bat for you in terms of getting more salary, more vacation or re-categorizing the job description so that the pay range is right..
