1. HR are sharks. Constantly hunting to fire employees. Contradicting rules and policies. HR has too much control on day to day. 2. Supervisors have zero clue of what actually goes on. Supervisors are under qualified. 3. Upper management (Directors, Managers, VP and up) unaware of lack of quality that happens in day to day operations. Upper Management let's HR do whatever they please. 4. Lack of education and teaching to employees. Rely way to much on tribal knowledge.
