Management sometimes digresses to pointing fingers, rather than concentrating on root cause and counter measure. Told continually not to make excuses when you bring issues to light that need to be corrected/investigated.

Poor organisational support. Glass ceiling at lower management or entry levels. Old fashioned and distant management style. Male dominated, old-boys club. Poor management communication , resistance to change and low levels of proactive initiative. Over reliance of managers on juniors to do their job and their managers job.
