Sometimes you get people who don't know what they are doing but they had the opportunity to be in a leadership position. Not everyone takes the internal review seriously. They don't even bother to provide feedback until annual review. There can be high turnover of staff at certain locations and the replacement may not know what happened. They get frustrated and leave.
