Silos ignorant of each other silo. Management ignorant of Operation. Production ignorant of Quality. Quality ignorant of Production. Lot of hours in a high-stress environment. Operators expect poor design from Engineers, so they don't report problems, and so they are never fixed. Too reliant on Contractors because turnover is so high. Company flattened management structure to concentrate the poor decision making, thereby limiting advancement opportunities to select "yes" people, which creates an extremely toxic work environment. Seems to be in a brain drain death spiral.
