the leadership team needs to really look at what people are really doing, HR in particular. HR WILL hold grudges and retaliate when someone point out they are doing wrong. If someone has decided a particular person is best in their eyes for a position, even if they are not a good fit, they will pressure hiring manager to hire them. I have witnessed employees that take credit for others work get promotions because they are the "social" one instead of the best one. they have also gotten rid of one supervisor because they did the right thing but kept a field supervisor that nearly ruined a district in the eyes of the customer and caused several techs to quit. Very inconsistent rules, and extreme favoritism, takes EEOC to the extreme, as in will only hire based on race or gender, not the best for the job.
