They ask for a cover letter stating why UCLA and why a specific area for your internship, and you must also send your CV. They also ask for your transcripts for undergrad and graduate school.
You are first placed in a group interview where they will ask you questions in a group, but individualy then you have second round individual interviews. There will be 3 managers interviewing you throughout the process and it is a bit intimidating at first, but the questions are not too bad.
