This role offers hands-on exposure to healthcare operations, leadership development, workflow management, and clinic coordination within a growing specialty neurology practice.
THE ROLE
Help patients feel prepared, supported, and cared for before they ever step into the clinic.
As a Patient Care Coordinator, you'll serve as a critical link between patients, providers, and clinic operations. Your primary responsibility is ensuring that every patient encounter starts with complete, accurate, and organized information.
You'll conduct patient intake interviews, gather medical history, coordinate outside records, prepare detailed pre-charts, and help ensure providers have everything they need before appointments begin.
This is a detail-oriented role that combines patient communication, clinical documentation, and operational coordination. Success in this position means providers spend less time searching for information, patients receive a smoother experience, and the clinic operates more efficiently.

WHAT YOU’LL BE DOING
Day-to-day, here's what this looks like.
Coordinate Patient Intake
- Conduct comprehensive intake interviews with new and returning patients in both English and Spanish.
- Guide patients through intake forms and accurately document information within the Electronic Health Record (EHR).
- Collect and verify patient demographics, insurance information, medications, allergies, and medical history.
- Gather detailed information regarding symptoms, chief complaints, treatment history, and relevant neurological conditions.
- Coordinate the collection of outside medical records, imaging studies, and supporting documentation.
- Ensure all required patient information is complete prior to scheduled appointments.
- Provide language support that helps Spanish-speaking patients clearly communicate their concerns and medical history.
Prepare Patient Charts
- Prepare comprehensive pre-charts for new patient consultations, follow-up visits, infusions, and procedures.
- Review and integrate outside records obtained through Care Everywhere and other medical record sources.
- Extract and document relevant laboratory results including TSH, Hemoglobin A1C, B12/Folate, LDL, and other provider-requested data.
- Review and summarize diagnostic imaging reports including MRI, CTA, MRA, VNG, and related studies.
- Update the History of Present Illness (HPI) using chronological patient information and prior visit documentation.
- Track pending referrals, testing, imaging studies, and laboratory results from previous visits.
- Utilize EHR templates, smart phrases, and clinic workflows to maintain consistency and efficiency.
- Ensure all charts are complete and provider-ready prior to clinic.
Support Patient Care Coordination
- Assist with insurance verification and prior authorization processes as needed.
- Manage referrals and coordinate communication between patients, providers, and outside facilities.
- Support appointment scheduling and follow-up coordination.
- Follow up on pending laboratory, imaging, and diagnostic test results.
- Coordinate record requests and communication with external healthcare organizations.
- Contact patients regarding appointment reminders, documentation requirements, and follow-up needs.
- Assist with operational projects, data management, and administrative initiatives that improve clinic performance.
Maintain Accuracy and Compliance
- Maintain accurate, detailed, and timely documentation.
- Follow HIPAA guidelines and protect patient confidentiality at all times.
- Adhere to provider preferences, clinic workflows, and documentation standards.
- Identify opportunities to improve processes, reduce delays, and enhance patient experience.
- Maintain professionalism, accountability, and empathy in every patient interaction.
